Back in London, Helen works to save a boy who is coding while his parents cry as they watch. Elizabeth and Floyd speak with a new mom about her twins and how they have tumors in their lungs. Max looks at fellow colleagues who have rashes. It seems the cheap gloves at the hospital are the cause.

Iggy works with a patient virtually. They play video games and bond. Iggy makes plans to meet him at a convention. He doesn’t get out much but agrees to meet Iggy. Meanwhile, Floyd and Elizabeth work on the twins who are still in utero. After they fix the problem, they cannot get the twins back in.

Helen learns thee boy she saved who is on a vent will be disconnected tomorrow, per a ruling from court. Back at New Amsterdam, Max talks to more coworkers and employees and learns more about the cuts they have endured since he left and came back.

Floyd and Elizabeth think about creating an ectopic pregnant for their patient since she doesn’t have enough room in the womb for her twins. Helen calls Max. She has an idea – she could transfer her young patient to New Amsterdam where his parents could keep him on a vent.

Dr. Walsh, who is now running the ED, asks for Laura’s help when a patient on a nitro drip continues to have chest pain with the drip too low but low blood pressure when the drip is too high.

Max helps a number of his coworkers. They hope he takes the hospital back from Veronica. He admits he is done and going home.

Iggy finally confesses to the young man he plays video games with that he is a doctor who is only trying to help him. The teen immediate disconnects the game they are in and disappears. Meanwhile, the British police intervene in Helen’s case, stopping the parents from bringing their son to the US. They are forced to say goodbye to him as he will be disconnected from life support.

Floyd and Elizabeth are thrilled the surgery on the twins were a success. Walsh tells Laura he wants her to take back the ED, he doesn’t like being the boss.

Iggy’s video game buddy comes in to see him and accepts his help. Iggy is thrilled. Laura offers Leyla a place to live while she fights for her visa. Max calls Helen to admit Veronica won until he hears chanting. The support staff has gone on strike. Max is overcome with admiration while they chant. Veronica turns in her badge.

Max comes to the roof. There is a celebration, they have the hospital back. Karen tells Max they need him back too. He has a lot to think about.
